Need a driver medical to get or renew your licence?

HGV / LGV / PCV D4 Driver Medicals 

If you’re applying for, renewing, or keeping a Group 2 licence (for lorries, trucks, buses, or coaches), the DVLA needs a completed D4 medical form. A doctor has to carry this out with you in person. 
 
You don’t need to go to your own GP — in fact, many charge more than private clinics like ours. 
 
Driver medicals are required  
At the outset, before driver training is undertaken, again at the age of 45y then every five years until 65 years of age.  
Annually thereafter from the age of 65  
under some additional specific circumstances, as specified on the official www.gov.uk website.

What’s included in your driver medical 

Your D4 medical checks whether you’re healthy and safe to drive a larger vehicle. It includes: 
A full review of your medical history 
Blood pressure check 
Vision test (with or without glasses/contacts) 
Urine test (to check for diabetes or other conditions) 
The doctor will fill out your D4 form during the appointment. We’ll even keep a copy on file (with your permission) in case the DVLA has any questions later. 

Important: Don’t forget your eyesight 

The number one reason people fail the driver medical is poor eyesight. 
 
If you’re not sure your eyesight meets DVLA standards, or haven’t seen an optician in the last 2 years, book an eye test first. If you wear glasses or contacts to drive, you’ll also need to bring your most recent prescription (dated within 12 months). 

What you need to bring 

To make your appointment as smooth as possible, bring: 
Photo ID – no ID, no medical 
Glasses / contact lenses (if you wear them) 
Your latest optician’s prescription (within 12 months) 
A list of any medication you’re taking 
GP or hospital letters if you have any ongoing conditions 
Be ready to give a urine sample during the medical 
Medical Examination form to print (Optional) 

Who can book a transport medical with Humber Med? 

You can book if you are: 
Applying for your first Group 2 licence 
Renewing your HGV, LGV, or PCV licence 
A company driver needing regular health checks 
Based in Hull, Beverley, or anywhere in Yorkshire or Lincolnshire (or further afield) 

Can you do taxi driver medicals? 

No — taxi medicals must be carried out by your GP, who can access your full medical records. Humber Med is not able to offer this.
